Get Ready for the Sports Season

Choose a convenient option to make sure your child is ready for middle or high school sports:

• A sports physical includes a physical exam with a provider to make sure your child is healthy enough to safely participate in a sport. The provider will complete your child’s sports physical form.
• A child wellness exam includes a comprehensive evaluation, vaccinations, family history review and a sports physical.

• Log-in: Get an e-visit or video visit with a physician
• Call-in: Talk to an experienced Sanford nurse
