Be Ready When the Whistle Blows

Complete your child’s sports physical at Sanford Worthington Clinic. Sports physicals are required to participate in middle or high school sports.

You have two options to complete a sports physical:

• Traditional sports physical. Includes an examination by a provider and the completion of your sports physical form.
• Comprehensive well child visit. Includes a sports physical exam, as well as a review and discussion of immunizations, family history, well-being and development. A well child visit is billed to insurance.

Please bring your child’s school sports physical form to your visit. The parent portion should be completed and signed.
