This Sanford "Playbook" includes specific guidelines, office-based and sideline evaluation tools, patient home instruction, and explanations of each state law. Anticoagulation Basics for Providers an Pharmacists

This course provides a basic overview regarding anticoagulation therapy. It addresses therapeutic ranges for Warfarin therapy; contraindications, warnings, and precautions of Warfarin and Enoxaparin therapy; and dosing protocols.
