Connect to Patient Health Records Online
One Chart Link is a web-based portal that extends read-only access to patient information within Sanford Health's electronic medical record, One Chart (powered by Epic), to referring providers and non-Sanford facilities.
Providers with One Chart Link access can review patient data for continuing care and treatment, place orders for a patient to a Sanford facility, view release of information reports and more.
To access One Chart Link, you must first complete the following documents and email them to us:
- Partner Agreement. This is a legal agreement between your facility/organization and Sanford Health and must be signed by an administrator.
- Link Site Administrator. This should be a main point of contact from your facility/organization. Once they have access, they will be able to electronically request additional staff from your site (support staff, non-Sanford providers).
- Form of Intent.
