What to Expect

Sanford Hospice care starts as soon as a request is made from the patient, the patient’s family or the patient’s doctor. Hospice care is usually ready to begin within 24 to 48 hours of the referral. In urgent situations, services may begin sooner.

Every hospice patient has access to an interdisciplinary team consisting of a: hospice volunteer, registered nurse, social worker, home health aide and chaplain. With the patient and family, each interdisciplinary team writes a care plan to ensure the patient and family receive the necessary services.

All visits are based on patient and family needs as outlined in the care plan and the patient’s condition during the course of the illness. Frequency of volunteer and spiritual care visits is often dependent on family request.
