Sanford AirMed Membership Program

Sanford offers an air transport membership program that helps keep the cost of healthcare down. This transport membership program helps by covering out-of-pocket expenses that occur with medically necessary air-medical transport. The membership program will directly bill the patient's insurance company and will cover any amount they do not pay. If the patient doesn't have insurance, this program will cover the entire cost of the medically necessary flight. The cost of this program varies from $59 for 12 months up to $1,000 for a lifetime membership.

STEP 2: Family Members

Sanford AirMed Membership covers husband and wife and unmarried, dependent children under the age of 23. Intensive Air considers your child a dependent if you claim him or her as a dependent on your income tax return. If you do not, a separate membership is necessary even if the child lives at your residence. Intensive Air Members will receive family financial coverage and membership cards.

STEP 4: Membership Agreement

  1. The Sanford AirMed Membership Program benefits are for myself, my spouse and dependent children under age 23 (those claimed on my income tax return) and listed on this form.
  2. Sanford AirMed is not an insurance company and the services provided under this agreement are not a secondary insurance coverage or a supplement to any insurance coverage. Sanford AirMed will accept payment from insurance carriers as payment in full.
  3. I transfer directly to Sanford AirMed my rights to air medical insurance payments due me. Such payments shall not exceed Sanford AirMed's regular charges.
  4. New member benefits take effect three days after receipt of completed enrollment and payment. "Elective" flight benefits take effect 30 days after enrollment.
  5. No refunds will be issued on membership purchases.
  6. Participation in the Sanford AirMed Membership Program may entitle me, or any of my above listed family members, to a 20% discount on air medical expenses incurred for an "Elective" medical flight provided only by Sanford AirMed. "Elective" transports are made by airplane and require scheduling and payment arrangements 24 hours in advance.
  7. Sanford AirMed flies based on medical need, not membership status, and transports patients to the closest medically appropriate facility of their choice.
  8. There may be times due to weather, back-up aircraft and crew, that Sanford AirMed cannot guarantee availability of service. AirMed is not responsible for payment of services provided by another ambulance service.

A card verification number is used to prevent fraud. It ensures that you, the cardholder, actually have the card in your hand. If someone else were to get their hands on your card number, they would not have your CVN.
