Aadil Rajwani, MD, is a family medicine hospitalist. He works with patients’ primary care providers and specialists to ensure they receive high-quality health care during a hospital stay.
Dr. Rajwani provides comprehensive medical care for a wide range of acute and chronic conditions. He offers a variety of diagnostic and therapeutic services to patients during their hospital stay, including:
- Diagnosing and treating common illnesses
- Facilitating discharge planning and smooth transitions to outpatient care, rehabilitation or home services
- Obtaining a patient’s medical history
- Ordering and interpreting labs, imaging and diagnostic tests to guide treatment
- Overseeing and adjusting medications during hospitalization
- Performing physical examinations
- Providing health education and counseling to patients and their families to explain conditions, treatments and care plans
- Providing preventive care in partnership with specialists, surgeons, nurses and the entire hospital care team
