Sanford Worthington Medical Center Home Care has served the Worthington area since 1985. Home Care provides a broad spectrum of services in the home for individuals who are recovering from an illness, disability or are chronically ill. In addition to professional nursing services and aide services, rehabilitation services such as Physical Therapy, Occupational Therapy and Speech Therapy take place right in the home. Homemaker services, which include assistance with cleaning and errands are also available.
Home telemonitoring provides information regarding your health status that is transmitted via the phone line to the home care office where it is monitored by a registered nurse 7 days a week. Health information that is transmitted includes blood pressure, pulse, weight, and oximetry (oxygen concentration in your blood). Having health information monitored daily allows for early intervention such as medication changes or additions which improve patient outcomes and may avoid hospitalization.
