Get your student ready for sports and other school activities during our sports physical event at the Sanford Worthington Clinic. A sports physical visit ensures your child can participate all year long.
This low-cost visit includes:
- A physical exam with heart and lung checks
- Review of injury and concussion history
- A check-in about lifestyle and habits
- Certain immunizations as needed (billed through insurance)
Please bring your $35 payment and signed sports physical form. Cash or cards are accepted. Parental permission is required for your child to receive a sports physical and/or immunizations.

Additional services provided during your visit that are not listed above will be billed separately to your insurance. If you prefer to file insurance for your appointment, please schedule a well child visit with your primary care provider. This comprehensive visit includes sports physical requirements and can be billed to your insurance with no payment necessary at the time of your appointment.
