This visit includes:
- A physical exam with heart and lung checks
- Review of injury and concussion history
- A check-in about lifestyle and habits
- Certain immunizations as needed (billed through insurance)
Please bring your $35 payment and signed sports physical form. Cash or card is accepted. Parental permission is required for your child to receive a sports physical and/or immunizations. Find an event near you below.
