Sports Physical Event at Sanford Health Lisbon Clinic

A sports physical visit ensures your child can safely participate all year long.

This visit includes:

• A physical exam with heart and lung checks 
• Review of injury and concussion history
• A check-in about lifestyle and habits
• Certain immunizations as needed (billed through insurance)

Dates: June 1 - September 1
Times: 8:30 a.m. - 4 p.m.

Please bring your $50 payment and signed sports physical form. Cash or card is accepted. Parental permission is required for your child to receive a sports physical and/or immunizations.
