Bridging Health and Home

Bridging Health and Home is a program offered to support older adults in our rural areas. Bridging Health and Home services are free for adults age 55 and older.

Our team of health care providers, nurses, staff and volunteers offer compassion and care to those wishing to remain at home as long as possible by providing:

  • Support from a health care team within your community
  • A bridge between your provider and managing health at home
  • Health and safety assessments, help with medications, and support for lifestyle changes

Services offered:

  • Answers to questions regarding new or old health concerns
  • Health screenings
  • Watching your blood pressure and blood sugar trends
  • Measures of your diabetes care (HgbA1C) and fats in your blood (Lipid panels)
  • Consults with a pharmacist for medicine reviews and health education
  • Dietary, Social Work, Physical and Occupational Therapy consults and education
  • Contact with your primary care provider
  • Connecting with Community resources
  • Better Choices, Better Health workshops
