How do I stay connected with my care team between appointments for my chronic condition?

Eligible patients with specific chronic conditions will receive weekly automated text messages to check in on their health. It only takes a few minutes to respond, and patients can pick a time that works best for them. If you are experiencing a decline in your condition or symptoms, a care team member will follow up and connect you to additional support. This program is not a replacement for how you currently communicate with your care team. Call 911 if you are having an emergency, or call 988 for the 988 Suicide & Crisis Lifeline.
