Home Care
We provide a wide range of services to the community including:
- Physical Therapy (Physical Therapists and Physical Therapy Assistants)
- Speech Therapy (Speech Therapists)
- Occupational Therapy (Occupational Therapists and Certified Occupational Therapy Assistants)
- Medical Social Services (Medical Social Worker)
- Home Health Aide
- Wound care for surgical wounds or a pressure sore
- Patient and caregiver education
- Intravenous (IV) or Nutritional therapies
- Medication education and management
- Assessing serious illness and unstable health conditions
- Establishing home exercise programs
- Assisting with therapeutic exercise
- Evaluating the home for safety and establish a safety plan
- Activities of Daily Living training or retraining
- Assisting with long range and community planning
- Assisting with dressing, grooming and bathing

Hospice
Hospice offers a multidisciplinary team that addresses the patient and their family in addressing physical, psychosocial, spiritual, and bereavement needs.
The hospice team’s goal is to ensure the best personalized quality of life for the hospice patient. Items addressed include but not limited to:
- Pain control
- Managing physical symptoms
- Assessing and anticipating changing needs
- Guidance with Advanced Care Planning, forms, and financial care needs
- Provide medications, durable medical equipment, and supplies needed to treat life limiting illness
- Support and teach family/caregivers how to care for the patient throughout the journey
- Provide bereavement counseling for up to 13 months after the patient has passed away
