Participation Agreement

Digital Experience Panel Participation

Sanford Health is conducting interviews and usability tests to improve digital tools and products. Your participation will help inform decisions about the design and functionality of these tools. 


Your Involvement
You may be asked questions about your experiences and opinions regarding specific products or tools. With your permission, we may record audio and/or video of your session and take written notes to document your feedback.


Your Participation is Voluntary
You may stop participating in a session at any time without providing a reason. If you choose to withdraw after completing a session, you may do so by contacting us. Upon withdrawal, any data or recordings associated with your session will be permanently deleted.


How We Use Your Data
Session recordings may be reviewed by other Sanford consumer experience team members for analysis and usability improvements. 

We may share findings with care delivery teams; however, your feedback will remain anonymous. No identifying information about you will be shared. 

If you are a Sanford patient, your participation is entirely separate from your medical care. Your data will not be linked to your medical record, and no Sanford caregivers involved in your treatment will have access to identifying information from your participation. 


Data Storage and Retention
Your personal information and session data will be stored securely and processed solely for experience improvement purposes. Once the team determines that your data is no longer needed, all recordings and related materials will be permanently deleted.
