Sports Physical Event at the Sanford Thief River Falls Clinic

A sports physical visit ensures your child can safely participate all year long.

This visit includes:

• A physical exam with heart and lung checks 
• Review of injury and concussion history
• A check-in about lifestyle and habits
• Certain immunizations as needed (billed through insurance)

Please bring your $35 payment and signed sports physical form. Cash or card is accepted. Your appointment cost is donated back to local school athletic programs. Parental permission is required for your child to receive a sports physical and/or immunizations.
