Sports Physical Event at Sanford Children's Southwest Clinic
A sports physical visit ensures your child can safely participate all year long.
This visit includes:
- A physical exam with heart and lung checks
- Review of injury and concussion history
- A check-in about lifestyle and habits
- Certain immunizations as needed (billed through insurance)
Dates: Tuesdays, July 7 - September 29
Time: 5:10 p.m. - 7:30 p.m.
Please bring your $50 payment and signed sports physical form. Cash or card is accepted. Parental permission is required for your child to receive a sports physical and/or immunizations.
