Sports Physical Event at the Sanford Thief River Falls Clinic

A sports physical visit ensures your child can safely participate all year long.

This visit includes:

• A physical exam with heart and lung checks 
• Review of injury and concussion history
• A check-in about lifestyle and habits
• Certain immunizations as needed (billed through insurance)

Please bring your $35 payment and signed sports physical form. Cash or card is accepted. Parental permission is required for your child to receive a sports physical and/or immunizations.
