Bismarck High School and Wachter Middle School Students
Get your child ready for sports and other school activities during our sports physical event at Bismarck High School. A sports physical visit ensures your child can participate all year long.
This $35 low-cost visit includes:
- A physical exam with heart and lung checks
- Review of injury and concussion history
- A check-in about lifestyle and habits
- Certain immunizations as needed (billed through insurance)
Please bring your payment, signed sports physical and exam consent forms. Cash and cards will be accepted.
Appointments are encouraged. Any student who is overdue for childhood immunizations may schedule a vaccine-only visit. If you have questions, call (701) 323-3700.
Sports physicals completed on or after April 15 meet requirements for the following school year.
Additional services provided during your visit that are not listed above will be billed separately to your insurance. If you prefer to file insurance for your appointment, please schedule a well-child visit with your primary care provider. This comprehensive visit includes sports physical requirements and can be billed to your insurance with no payment necessary at the time of your appointment.
*Students from schools not listed above, or those who are unable to attend their designated school's event, may attend any of the sports physical events happening in Bismarck-Mandan this June.
