Bismarck High School
Get your required school vaccinations at the Bismarck High School. We’ll ensure you get the appropriate vaccines based on your age and immunization history.
The parent or guardian must complete the consent for immunization and questionnaire prior to the student receiving vaccines. The form can be brought with the student on the day of immunizations or sent electronically to Kate_Gartner@bismarckschools.org.

On the day of the event, wear a shirt with sleeves that can be easily rolled up and bring your insurance card. Most insurances are accepted.
