How Does it Work?
Your physician or social worker can refer you to our home care team. We will them meet with the patient and/or family to discuss personal needs. Next, we develop a coordinated care and treatment plan. It’s our goal to have someone at your home within hours of being referred to us.
We provide services 24 hours a day, seven days a week – we’ll be there for you when you need us.
Who Pays for Home Care?
The cost of home care depends on what services are provided. Medicare, Medicaid, VA, private insurance and workers compensation often cover many of the costs of home care. We will discuss with you the costs and payment options. For services covered by a third party, Sanford Home Care will complete the necessary claim forms and bill the third party directly.
What if I Need Additional Medical Care?
Sanford Health home care specialists have direct access to physicians, specialists, equipment and expertise to ensure that you receive the very best in care. If your medical condition changes and requires hospitalization, we can arrange your transportation.
