Sanford Clinic Canton has been awarded a two-year federal grant by the Health Resources and Services Administration (HRSA). The grant dollars will allow the clinic to hire a healthcare coach who will assist patients who have been diagnosed with either diabetes or cardiovascular (heart) disease.
Through the grant dollars, the clinic will also be able to purchase a patient registry software program that can help identity patients who could benefit from visits with the healthcare coach, track the patients’ progress, provide community education and health screening opportunities and report outcomes on diabetes and CVD indicators to the Office of Rural Health.
Dawn Croatt, Clinic Director at Sanford Clinic Canton says, “The medical care and professional guidance that the coach will provide each patient will be in addition to the great care the patient is already receiving from his/her primary care physician at the clinic.”
The ultimate goal with the grant is to improve patient care and serve as a model program for other clinics in the Sanford Health system, as well as other rural clinics and medical providers.
