(Fargo, ND) – Sanford Medical Center Fargo has received the American College of Cardiology Foundation’s National Cardiovascular Data Registry (NCDR) ACTION Registry–Get With The Guidelines (GWTG) Gold Performance Achievement Award for 2011 – one of only 167 hospitals nationwide to do so. The award recognizes Sanford’s success in implementing a higher standard of care for heart attack patients, and signifies that Sanford has reached an aggressive goal of treating patients to standard levels of care as outlined by the American College of Cardiology/American Heart Association (ACC/AHA) clinical guidelines and recommendations. To receive the Gold Performance Award, Sanford consistently followed the treatment guidelines in for 8 consecutive quarters and met a performance standard of 85% for specific performance measures. Following these treatment guidelines improves adherence to ACC/AHA Clinical Guidelines, monitors drug safety and the overall quality of care provided to heart attack patients. “The ACC Foundation and AHA commend Sanford for its success in implementing standards of care and protocols. The full implementation of acute and secondary prevention guideline-recommended therapy is a critical step in saving lives and improving outcomes of heart attack patients,” Gregg C. Fonarow, MD, NCDR ACTION Registry-GWTG Steering Committee Chair. “Sanford has been, and continues to be focused on improving the quality of cardiovascular care. The number of acute heart attacks patients eligible for treatment is expected to grow over the next decade due to increasing incidence of heart disease and a large aging population,” said Dennis Millirons, president, Sanford Medical Center Fargo. The Foundation is a partnership between the ACC Foundation and the AHA with partnering support from the American College of Emergency Physicians, Society of Chest Pain Centers and the Society of Hospital Medicine. The Foundation empowers health care teams to consistently treat heart attack patients according to the most current, science-based guidelines and establishes a national standard for understanding and improving the quality, safety and outcomes of care provided for patients with coronary artery disease, specifically high-risk heart attack patients. BIOBUSINESS BRIEFS Sanofi–Aventis has signed an agreement with CureDM, entitling Sanofi–Aventis to exclusive worldwide rights to develop and market Pancreate — a first-in-class peptide therapeutic that may have the potential to stimulate pancreatic islet neogenesis — for the treatment of type 1 and type 2 diabetes. The agreement for this preclinical-stage agent involves an upfront payment and additional milestone payments, potentially totalling US$335 million. “In both type 1 and late-stage type 2 diabetes, a fundamental problem is the loss of functional islet β-cell mass, resulting in insulin deficiency and hyperglycaemia,” explains Paul Burn, Chair and Director of The Sanford Project, and professor at the Sanford School of Medicine, University of South Dakota, USA. “Although insulin therapy is a life-saving treatment for type 1 diabetes, and has a key role in the treatment for patients with advanced type 2 diabetes, it cannot prevent the development of long-term complications,” he adds. “So, the development of disease-modifying therapies — capable of restoring functional β-cell mass and endogenous insulin production — remains a major unmet medical need.” Several strategies aimed to address this need are being actively pursued. “Transplantation of either whole pancreas or islets is one approach, although it is not widely applicable because of the limited numbers of organ donors and the significant negative long-term consequences of the necessary immunosuppression,” says Lawrence Rosenberg, professor at McGill University, Quebec, Canada. “Moreover, it is not suitable for individuals with type 2 diabetes — the vast majority of the diabetic population.” Therapeutic strategies capable of stimulating or copying the body’s own repair mechanisms and mimicking the actions of endogenous peptide and protein factors are also receiving significant attention, says Burn. Although islet neogenesis mainly takes place only during fetal development, it can occur in adults in response to pancreatic injury or stress, and it seems that this process may be exploited therapeutically. “Islet neogenesis associated protein was the first and is now the most advanced example of such agents, and is currently in a late Phase II clinical trial,” notes Rosenberg. Burn also highlights other approved agents that have been reported to have beneficial effects on functional β-cell mass, including glucagon-like peptide 1 analogues, gastrin, dipeptidylpeptidase 4 inhibitors and proton pump inhibitors (Nature Rev. Drug Discov. 9, 187–188; 2010). CureDM’s Pancreate (human islet peptide), a 14 amino-acid peptide encoded by a portion of the human regenerating islet-derived 3α gene, is thought to stimulate islet neogenesis by triggering the differentiation of pancreatic progenitor cells. So far, preclinical studies have reported that Pancreate increased insulin production in human pancreatic ductal cell cultures and improved glycaemic control. Additionally, it increased islet numbers and normalized diabetes markers in mice with streptozotocin- induced diabetes (Endocr. Pract. 14, 1075–1083; 2008). Although Rosenberg thinks that as a better understanding of the mechanisms of islet neogenesis are elaborated, approaches to regenerative therapy of diabetes will undoubtedly take root, for Pancreate he cautions that: “There appear to be little robust data on which to evaluate its potential for use in humans and moreover, a safety profile has yet to be established.” Phase I trials for Pancreate are expected to begin later this year.

FROM THE ANALYST'S COUCH Type 1 diabetes (T1D), also termed juvenile-onset or insulin-dependent diabetes, is an autoimmune disease and a metabolic disorder characterized by T-cell-mediated destruction of pancreatic β cells, resulting in insulin deficiency and hyperglycaemia. In 2007, it was reported that 437,500 children were affected by T1D worldwide1. About 70,000 children aged under 14 years are developing T1D per year1, with a reported annual global increase of about 3%, particularly in younger children2. The aetiology of T1D is largely unknown, but it is thought that a genetic predisposition, environmental factors and distinctive metabolic changes are involved in the initiation, development and progression of the disease. Insulin deficiency in T1D leads to increased gluconeogenesis and lipolysis, elevated metabolism of free fatty acids, and the generation of ketone bodies, resulting in diabetic ketoacidosis. The primary clinical signs of T1D are ketoacidosis — which can lead to coma and death — and chronic hyperglycaemia. Chronic hyperglycaemia is the primary cause of several macrovascular and microvascular diabetic complications, including cardiovascular disease, renal disease, diabetic retinopathy and peripheral neuropathy. Therapy and unmet medical needs Insulin-replacement therapy is the life-saving first-line treatment for T1D, and the pharmaceutical industry’s focus has traditionally been on developing novel insulins with improved pharmacokinetic profiles, more convenient formulations and alternative delivery methods. For the seven major pharmaceutical markets, the number of diagnosed and drug-treated cases of T1D is estimated at around 1.84 million (FIG. 1), resulting in an approximate market size for insulin sales of US$2 billion for the T1D indication (FIG. 2). However, although intensive insulin therapy for T1D is life-saving, it is not a cure. It can result in hypoglycaemic incidents and cannot prevent the development of long-term complications. So, the development of disease-modifying therapies remains a major unmet medical need. Therapies for patients with newly diagnosed (recent-onset) as well as established T1D are needed. Immunological approaches. At present, the most advanced disease-modifying strategies are aimed at halting the further progression of T1D by inducing immune tolerance or by modulating the autoimmune and/ or secondary immune and inflammatory responses. Antigen-specific, vaccine-based approaches using β-cell-specific autoantigens to induce immune tolerance are under development and include glutamate decarboxylase 65 kDa isoform (GAD65; also known as GAD2) and BHT-3021 (Table 1). Non-antigen-specific immunomodulators (Table 1), such as the CD3-specific monoclonal antibodies otelixizumab and teplizumab are also in development. However, owing to their mechanism of action, these agents might be more prone to compromising the normal function of the immune system. In addition, various non-antigen-specific, FDA-approved immunomodulators are being explored in clinical research settings for their utility in T1D by TrialNet, the Immune Tolerance Network and academic institutions. Although such strategies could delay or halt the further decline of β-cell function, patients with T1D typically only retain about 10–20% of functional β-cell mass at the time of disease diagnosis. Thus, vaccine-based, tolerogenic approaches have the potential to be most effective in non-diabetics who are at risk of developing T1D. Such approaches may be less effective in patients with recent-onset or established T1D who have lost most of their β-cell mass already and so require insulin-replacement therapy to maintain glycaemic control. Similarly, nonantigen- specific immunomodulators may be most effective when given early in the disease process, before the onset of clinical overt diabetes. Overall, based on safety considerations of non-antigen-specific therapies and/or the limited patient population who may benefit from any particular approach, the market size of any of these potential immunological products for T1D is likely to be limited (FIG. 2). Regenerative approaches. β-Cell mass expands in response to increased metabolic demand such as in pregnancy and obesity. It also has been established that β cells can replicate, differentiate or transdifferentiate from various endocrine or non-endocrine cells, thus providing hope that reduced β-cell mass can eventually be restored endogenously. More specifically, it has been shown that certain peptides and growth factors such as gastrin and glucagon-like peptide 1 (GLP1) can increase β-cell mass and restore normoglycaemia in animal models of T1D in the absence of immunosuppressants. Two FDA-approved drugs, a dipeptidyl peptidase 4 (DPP4) inhibitor and a proton-pump inhibitor, can elevate circulating levels of GLP1 and gastrin in diabetic NOD mice5, thereby providing the rationale for testing these approaches in humans. Cellular approaches including stem cell, progenitor cell, dendritic cell and xenotransplantation are also providing scientifically exciting opportunities, although most are at an exploratory stage. Moreover, in addition to the scientific and technical challenges, there are considerable regulatory hurdles associated with cell-based therapies that need to be addressed before commercially viable products move towards the market. Combination therapies. Although an attractive approach for treating patients with recent-onset or established T1D, any regenerative therapy by itself may be hampered by the uncontrolled, ongoing autoimmune response. So, combining safe, antigen-specific tolerogenic therapies with emerging, regenerative therapies seems to be the logical next step in the research and development of novel disease-modifying medicines. Pregnancy is associated with a mild suppression of the maternal immune system to tolerate the developing fetus, and the expression and action of factors that stimulate growth and regeneration. A recent study reported that women with long-term T1D show a pregnancy-induced improved β-cell function and an associated improvement in glycaemic control7. This provides support to the idea that mimicking nature by combining safe tolerogenic agents with regenerative therapies might have the potential to become an important strategy for developing disease-modifying treatment regimens for T1D. Market Evolution On the basis of the current T1D development pipeline, insulin-replacement therapies will dominate the market for T1D over the next decade (FIG. 2). In the long term, the growing, but early, pipeline of novel tolerogenic, antigen-specific and β-cell-specific regenerative agents could provide a promising platform for the development of disease-modifying therapies. Although single-agent therapies are likely to reach the market first, combination therapies could be most effective in delivering the long-sought cure. A major illness is a scary enough proposition for an adult, but for a sick child who may be too young to fully understand what is happening, it can be especially hard. In this regard, children’s hospitals serve several functions, including specialty medical center, research organization and supportive community for families in need. Here are some up-and-coming children’s hospitals that excel in these areas, whether advancing disciples like pediatric neurosurgery, racing for a cure for type 1 diabetes, gaining a greater understanding of autism, or by building idyllic sanctuaries that captivate the imaginations and calm the nerves of adults and children alike. Castle of Care Sanford Children’s Hospital in Sioux Falls, S.D., is a freestanding pediatric center and “Castle of Care” that just opened its doors in early 2009. The facility takes its name from the hospital’s architectural design and interior, which is all tailored to look like a medieval castle, complete with towers, thrones, tapestries and almost 4,000 square feet of murals depicting fairytale lands. There is also a large “fireplace” in the great hall that creates the illusion of fire with child-safe water vapor diffused with red lighting. On every floor of this 5-story, 146-bed pediatric hospital is a different theme played out in the décor, from floor to ceiling and in murals in every child’s room. Wards like “The land of Nez” and “The Enchanted Forest” promote a playful, non-threatening atmosphere that can mean a little less stress and potentially faster healing for patients. “Over the last 20 years, we’ve talked about what we need to provide for the children in our community,” says Jan Haugen-Rogers, vice president of Sanford Children’s Hospital. “We knew it was a once-in-a-lifetime opportunity— a foundation for the future. We thought about our core services and what the future holds and we wanted to strategize and do it right the first time. Health, hope, healing and strength became the theme, and we thought, ‘What’s a building that is timeless from an architectural standpoint that would truly withstand all time – what about a castle?’” A lot more than just design is happening at this hospital. About 65 percent of the patient population at Sanford Children’s is under age 5, with a slightly older age group receiving care in the state’s only dedicated oncology program. Also under Sanford’s roof are both neonatal and pediatric intensive care units and a full radiology department, complete with hybrid PET/CT system. More than 60 physicians work in more than 20 pediatric subspecialties, ranging from orthopedic surgery to endocrinology and cardiology to genetics. Haugen-Rogers says there could even be some potential for proton therapy in the future. The gift that keeps on giving All this was made possible by donations from the community, including a generous $16 million gift from Dennis “Denny” Sanford, a prominent businessman and philanthropist in the region. This gift pales in comparison to Sanford’s even more generous gift of $400 million, which will fund a number of programs involved in the larger Sanford Health System. Sanford Health is the largest integrated health network in the area and home to several research programs, including the Sanford Project, which is all about finding a cure for type 1 diabetes – within the benefactor’s lifetime. This is Sanford Children’s flagship research program. Ruth Krystopolski is vice president of growth and development for Sanford Health and has overseen this project since its inception. Sanford wanted his funding to go toward a cure, but it was up to a team of experts and researchers to determine the target and trajectory of the research. “First we had to identify and select a disease focus, and we needed something that we could find a potential cure for within Mr. Sanford’s lifetime,” says Kystopolski. Researchers looked at incidence rates and funding models and narrowed down disease processes and then wrote full white papers on all potential contenders. With diabetes mellitus presently affecting an estimated 500,000 to 1 million Americans, and with more children at risk of developing this disease than most other chronic illnesses, it stood out from all the rest as one of the most serious and potentially the most curable pediatric diseases. In June of 2008, it was announced by the expert advisory panel that type 1 diabetes would be the focus of the Project. Now that the project is underway, the Sanford Children’s Hospital is recruiting leaders in diabetes research and is getting ready to move research operations into a separate 300,000 square foot facility early next spring. “There is a lot of energy and time being spent doing background on experimental and translational research in both in vivo and clinical models and pharmacology,” says Krystopolski. Dr. Paul Burn is one of those leading researchers heading up the Project. Burn is an M.D. and professor of pediatrics at the University of South Dakota Medical Center and is also senior vice president of research and development for the Juvenile Diabetes Foundation. His work with Sanford Children’s Hospital and the Sanford Project is focusing on the regeneration of beta cells — the cells involved in the production of insulin — as well as the immune response that triggers the disease. “Sanford’s research into beta cell regeneration is just getting started,” says Dr. Burn. “Our hope is that we can take what we learn to the patients, because we feel like we have a unique opportunity to translate that to an underutilized and relatively stable patient population.” Regeneration for the new generation Type 1 diabetes is usually diagnosed in children from birth up to the first two decades of life. In this autoimmune disease, the immune systems of diabetes patients mistake pancreatic beta cells as a foreign threat, and begin destroying those cells. If all the beta cells are destroyed, the body is no longer able to produce insulin to regulate blood sugar levels. “Injecting insulin is not really a cure for the disease,” says Burn. “You are just replacing what the body can no longer provide for itself. If you want to interfere with that disease process you have to moderate the immune system so that the immune system no longer attacks those beta cells.” Dr. Burn and his growing team of researchers are getting a jump-start on the Project—and the cure—by researching a novel pharmaceutical approach to beta cell regeneration using the pill-form of already FDA-cleared drugs, namely a protein called GOP-1 and gastrin, both of which are currently used for other indications. “Those two drugs have been shown to endogenously elevate two factors that are responsible for regenerating beta cells and may be having an effect on the immune system, as well,” says Dr. Burn. In March of 2010, Sanford Children’s will prepare the investigational new drug and apply with the FDA to begin clinical trials. If successful, Sanford Children’s could be on the fast track to a cure. If all goes well, the treatment might one day also be applied to type 2 diabetes sufferers. “That is our big hope at the present time,” says Dr. Burn. “Because we are using already-approved drugs, should our trial be successful, physicians could immediately start using those drugs to treat patients, whereas it can take 15 years and $1 billion to go through the entire drug development process. We should be in position to give the whole field a jump-start.” The first trial is modest, including about 56 patients who will be given a combination of those two drugs or a placebo for one year. These young patients are coming into the trial newly diagnosed and fresh out of the doctor’s office. This is what Krystopolski refers to as research that goes “from bench to bedside” and can be seen in real patient results. These children have most likely already lost 90 percent of their beta cells, but are still at an optimal stage for beta cell regeneration. Burn says that it may be more difficult for adults to regenerate the cells or moderate the immune system with the same success. The results of the first trial will be analyzed and evaluated and should be presented by the end of the second year. Guiding pediatric neurosurgery At the Montreal Children’s Hospital at the McGill University Health Center in Canada, some new technology is taking this 105-year-old children’s hospital to new heights. The institution has already built a name with its work in genetic diseases, cardiology and pediatric surgery, but it is now carving out a space for image- guided pediatric neurosurgery. Dr. Jean-Pierre Farmer is chief of the newly created Department of Pediatric Surgery at the hospital and has overseen the recent overhaul of the hospital’s OR to make room for some very advanced technology: a new Philips Achieva 3.0T X-series MRI system combined with BrainLAB’s BrainSUITE iMRI intraoperative platform. With the high-field MRI scanner, digital intraoperative navigation system and custom OR components, neurosurgeons at Montreal Children’s are able to conduct image-guided neurosurgery with cutting-edge precision. The new MR/OR is already in operation and is being used predominately for brain tumor resection and surgical treatment for epilepsy. Since the early 1990s, neurosurgeons have been performing various forms of image-guided neurosurgery. Usually brain scans are taken the night before or sometime prior to surgery and those scans are used to map out the surgical plan, but “occasional retraction and brain shifts” can happen during the operation, requiring reevaluation. “That is the reason for introducing intraoperative MRI—to be able to update the system when you critically need it,” says Dr. Farmer. “The accuracy is very, very good. If you have been able to do functional MRI fiber tracking on the patient before, you are able to fuse images with this new information and update your scan.” Dr. Farmer says Intraoperative MRI is also great with ear, nose and throat surgeries and orthopedic surgeries, and can be particularly helpful in spinal cord surgeries, assuring that surgeons are not dissecting too much or too little of this critical anatomy. The use of MR is also picking up for pediatric facilities, as frequent use of CT may be too risky for this patient population. “There is now a trend in pediatrics for using MR rather than CT,” says Farmer. “There is some concern about cumulative radiation dose from repeated CT scans.” Montreal Children’s is the first hospital in Canada and the first pediatric hospital to bring a 3T magnet into the OR. To complete the set-up, the OR was brought into the radiology department, where sliding doors connect the MR system to the OR and the iMRI docking table serves as both an OR surgical table and MRI couch. The whole interlocking system maintains sterile conditions fit for surgery. The high-field MRI is multipurpose. When not in use during surgery, the Achieva is being used by researchers studying autism and cerebral palsy. Farmer says that the new imageguided neurosurgery system is just a sign of what’s to come. That’s not the only developing news for the future though. The children’s hospital is going to be moving into new a home within four years’ time. “This project is a glimpse into the future,” says Dr. Farmer. “About 70 percent of this will be moved into the new facility.” The new location will help the hospital build a bridge with the McGill University Health Center to better utilize the resources there. Upon completion, Montreal Children’s will gain access to a new cancer pavilion and radiation oncology department, as well as a women’s health pavilion, where babies can be delivered on site. America’s next top pediatric research hospital Children’s Healthcare of Atlanta is also moving up in the area of pediatric research. The Atlanta, GA-based institution has been working to enhance its research for several years and has built a strategic enterprise of researchers from collaborating institutions, including nearby Emory University and Georgia Institute of Technology. In March of 2009, Dr. Paul Spearman was named chief research officer for Children’s Healthcare of Atlanta and vice chair of Emory School of Medicine, and just recently received a $30 million grant from the Joseph B. Whitehead Foundation to help Children’s advance their research organization and to fund continuing research in autism. “This partnership is moving along quite quickly,” says Dr. Spearman. “We are focused on developing child-health related research and the $30 million donated from the Joseph B Whitehead Foundation will enable us to build a brand new research building, and $5 million of that will be going to develop Children’s Marcus Autism Center.” Children’s already has the Aflac Cancer Center (yes, sponsored by the insurance provider Aflac), but will also be focusing on the development of seven new research centers—in immunology and vaccines, transplant immunology, engineering and translational research, cystic fibrosis, developmental lung biology, cardiovascular biology and endothelial cell biology. In addition to grants from the community, Children’s has also received $12 million from the National Institutes of Health to drive the hospital’s next generation of research. “We have a plan to become a topten research institute by 2018,” says Spearman. “This funding will really enable us to be able to do that. We already have a number of research strengths and will be reorganizing our pediatric focus areas. It’s a very exciting time.” The Marcus Autism Center is also being staged for future research. “That element is still at an early stage, but we have a very, very strong commitment,” says Spearman. “We know that the incidence of autism is increasing. The Center will be a place to learn more about autism and what causes it.” Additional research centers will be focusing on pediatric neurosciences, and clinical and translational research, and other organizations, like the Center for Drug Discovery and Outcomes and Wellness Research Center, will be sure to take Children’s to the next level as a competitive pediatric institution. Into the future The Sanford Children’s Hospital, Montreal Children’s Hospital and Children’s Healthcare of Atlanta all represent a new wave in pediatric facilities, from design and function to research and treatment. Innovative architecture, advanced technology and a commitment to exploring and finding new ways to treat—and cure—pediatric disease is what it’s all about.

Generalized anxiety disorder (GAD), one of many different anxiety disorders, is characterized by excessive anxiety and worry about a number of events and activities, such as work or school performance. In GAD, anxiety and worry are associated with specific symptoms, including restlessness or feeling keyed up, uptight, or on edge; being easily fatigued or feeling 'drained'; having difficulty concentrating or feeling that one's mind has gone blank; irritability out of proportion to whatever may have caused it; and feeling angry for no apparent reason. Muscle tension or tightness occurs, as well as sleep disturbances, especially difficulty falling or staying asleep or having a very restless, unsatisfying sleep. For GAD to be diagnosed, the intensity of anxiety has to cause some impairment in the individual's ability to function either on the job or in social relationships. The anxiety may also cause physical symptoms including shortness of breath, chest tightness, rapid and pounding heartbeat, sweating, a sensation of choking, and abdominal distress. Some forms of anxiety besides GAD include panic attacks, in which the physical symptoms just mentioned occur 'out of the blue' for no apparent reason, last for a very brief period, and then resolve. The individual with panic attacks may not, in fact, be anxious most of the day and may be relatively calm between the episodes of panic. Agoraphobia, another form of anxiety, is a fear of being out in open spaces alone, where one might feel trapped and unable to get home. Agoraphobia often occurs in conjunction with panic and sometimes leads individuals to become virtual prisoners in their own homes. Phobias are a type of anxiety involving fears of specific objects, places, or behaviors. Examples of phobias include fear of urinating in public restrooms, fear of using public transportation, fear of heights (acrophobia), fear of foreigners (xenophobia), and fear of closed-in places (claustrophobia). Other forms of anxiety include the obsessive-compulsive disorder, posttraumatic stress disorder, and acute stress disorder, as well as the anxiety caused by legal drugs such as caffeine, the anxiety caused by drugs of abuse such as amphetamines or cocaine, and the anxiety caused by medical conditions and medications, such as those used to treat asthma (steroids, aminophylline). In general, anxiety is a state of fear or worry that: May or may not have a cause. The individual cannot control. That significantly compromises the individual's ability to function normally. It should be pointed out that worry and anxiety are normal feelings; however, it is all a matter of degree. Sometimes anxiety can allow us to make plans and provisions for the future and can, in fact, be beneficial. Such beneficial anxiety is called anticipation. Living With Your Diagnosis Generalized anxiety disorder is fairly common, affecting up to 10% of individuals at any particular point in time. Its childhood equivalent, the so-called overanxious anxiety disorder of children is also fairly common. Studies of families suggest that anxiety can be transmitted to children genetically, especially in conditions such as panic disorder. The matters about which anxious patients can worry are endless. They are likely to report worry over minor matters, and they are often anxious for at least half the day during an average day. In children and adolescents, the worries will center around the quality of their school performance or some aspect of their social functioning in school. They may also be concerned with their own physical or mental imperfections as they see them, and such anxious adolescents will require constant reassurance. There are some medical conditions that have a high correlation with anxiety. These include such conditions as ulcerative colitis, Crohn's disease, asthma, hypertension, heart disease, ulcer disease, reflux esophagitis, and headaches. During evaluation, anxious patients often have rapid or pressured speech and often shift from one subject to another without any apparent connection. These patients may be extremely restless, shifting about in their chair or tapping their fingers or toes, ringing their hands, putting their head in their hands, and often even getting up and walking across the room. Patients will use such phrases as 'I feel like I'm going to jump out of my skin,' 'My whole body's on fire,' 'I think I'm going to have a heart attack,' or similar comments. In the treatment of anxiety, it is important to determine whether some medical condition or substance abuse is causing the anxiety. Common drugs that can produce anxiety include theophylline, any medications with caffeine, steroids, many antihypertensives including Aldomet, stimulating antidepressants such as Prozac, inhalers used for breathing problems such as Brethine and Vanceril, thyroid medication, and diet pills. Many over-the-counter medications such as some antihistamines, some cough and cold preparations, and diet pills that contain caffeine can also cause anxiety, and dietary intake of excessive amounts of caffeine and sugar can make any anxiety syndrome worse. Once a medication or a medical condition has been eliminated as a cause of anxiety, then an adequate history should be obtained for substance abuse to eliminate the possibility that the individual may be using some kind of psychostimulant that might be producing anxiety. Attention should also be directed toward uncovering any precipitants in the individual's home environment or any major stressors that might be contributing to the anxiety. Treatment The treatment of anxiety involves both behavioral techniques and medication. One behavioral technique used is biofeedback, wherein patients are hooked to a machine and learn to decrease their muscle tone or control their brain waves by regulating their breathing. Other behavioral techniques include progressive muscle relaxation, which is often done to a prerecorded tape; imagery, where individuals imagine that they are in some pleasant setting; meditation; and hypnosis. Behavioral techniques have been very effective in treating anxiety and are the commonly used methods in those patients who prefer not to take medication. If you have been prescribed a medication for anxiety, it is most likely one of the minor tranquilizers of the benzodiazepine class. This would include such drugs as alprazolam (Xanax), lorazepam (Ativan), or diazepam (Valium). Although these drugs are effective for the rapid relief of anxiety, they do have side effects. They typically slow down breathing and therefore may not be the best drugs to use in someone who has asthma, bronchitis, or emphysema. They also are broken down by the liver, so they may be bad choices in someone who has severe liver disease, such as cirrhosis or hepatitis. Finally, these drugs can be habit forming; that is, they cannot be discontinued without being tapered for fear of withdrawal signs and symptoms. Those individuals who have a history of substance abuse, particularly abuse of depressant drugs, such as alcohol, barbiturates, or benzodiazepines should not be prescribed these drugs. Because the duration of action of some of the benzodiazepines, such as Xanax and Ativan, is fairly short, they may have to be given three or four times a day. Also, the benzodiazepines may produce a significant degree of sedation, which can impair driving and the ability to operate certain machinery. Another group of drugs that are used to treat anxiety are the antidepressants. The tricyclic antidepressant drugs, such as imipramine, have been very effective for years in treating anxiety disorders. The major drawback of these medications is that they are not effective as quickly as the benzodiazepines. It may take 10 days to 2 weeks before the beneficial effects of imipramine and other tricyclic antidepressants are seen. They also may initially increase anxiety before relieving it, and they have side effects such as weight gain, sexual dysfunction, dry mouth, constipation, and blurry vision. The advantages of the tricyclic antidepressants are twofold: They can be given once daily. They may be more effective than the benzodiazepines when depression is associated with the anxiety, as it often is. Other antidepressants used to treat anxiety are the selective serotonin reuptake inhibitors (SSRIs). In particular, paroxetine (Paxil) seems to have significant antianxiety effects. It can be given once daily, usually in the evening, and may improve sleep. It has few side effects, except for diarrhea, constipation, and some sexual side effects. Paxil is similar to imipramine in that its therapeutic onset is delayed; it may take 2 to 3 weeks before he beneficial effects are seen. In addition, you may have to avoid using Paxil if you are taking certain other medications because of its interaction with them. Buspirone (BuSpar) is also used for anxiety. It is the only drug approved for anxiety that is not potentially physically addicting, so it is often substituted for the benzodiazepines. The advantages of using BuSpar are that it is less likely to cause sedation and that there is no withdrawal on discontinuing it. Side effects of BuSpar may include gastrointestinal distress and headaches. Finally, the beta-blocker, propranolol (Inderal) is often used for treating some of the effects of anxiety and is particularly effective for treating the runaway heartbeat and sense of heart pounding that many anxious patients feel. Because Inderal is also used to treat high blood pressure, individuals with low blood pressure should not take it. It can also sometimes make individuals feel very tired, and should not be used in patients who have severe lung disease. Anxiety disorders can cause significant suffering and worry for patients. However, these are treatable conditions. The DOs If you have an anxiety disorder diagnosed, it is very important to minimize your level of stress; to have some activity that you enjoy doing such as reading, writing, or knitting; to participate in a regular exercise program; and to watch your diet. If you are taking antianxiety medications, you should be very careful when driving or operating dangerous machinery. The DON'Ts Do not use products containing caffeine, and decrease your sugar intake as much as possible. In addition, remember that most prescribed medications for anxiety have some sedating effects; therefore you should avoid drinking alcohol. When to Call Your Doctor You should call your doctor if the nature of your anxiety changes, if you notice any side effects from your medications, or if your anxieties are accompanied by depression and suicidal ideation or thoughts. Although just enough stress can be a good thing, stress overload is a different story - too much stress isn't good for anyone. For example, feeling a little stress about a project or family event that's coming up can motivate you to focus or work hard. But stressing out too much over the project or event can make it hard to concentrate on the tasks and what to do first. Pressures that are too intense or last too long, or troubles that are shouldered alone, can cause people to feel stress overload. Here are some of the things that can overwhelm the body's ability to cope if they continue for a long time: being bullied or exposed to violence or injury relationship stress, family conflicts, or the heavy emotions that can accompany a broken heart or the death of a loved one ongoing problems with work related to a work overload, conflicts with colleagues or job dissatisfaction crammed schedules, not having enough time to rest and relax, and always being on the go Some stressful situations can be extreme and may require special attention and care. Posttraumatic stress disorder is a very strong stress reaction that can develop in people who have lived through an extremely traumatic event, such as a serious car accident, a natural disaster like an earthquake, or an assault like rape. Some people have anxiety problems that can cause them to overreact to stress, making even small difficulties seem like crises. If a person frequently feels tense, upset, worried, or stressed, it may be a sign of anxiety. Anxiety problems usually need attention, and many people turn to professional counselors for help in overcoming them. Signs of Stress Overload People who are experiencing stress overload may notice some of the following signs: anxiety or panic attacks a feeling of being constantly pressured, hassled, and hurried irritability and moodiness physical symptoms, such as stomach problems, headaches, or even chest pain allergic reactions, such as eczema or asthma problems sleeping drinking too much, smoking, overeating, or doing drugs sadness or depression Everyone experiences stress a little differently. Everyone experiences stress at times - adults, teens, and even kids. But there are things you can do to minimize stress and manage the stress that's unavoidable. Stress is a feeling that's created when we react to particular events. It's the body's way of rising to a challenge and preparing to meet a tough situation with focus, strength, stamina, and heightened alertness. The events that provoke stress are called stressors, and they cover a whole range of situations - everything from outright physical danger to making a presentation or juggling multiple life roles. The human body responds to stressors by activating the nervous system and specific hormones. The hypothalamus signals the adrenal glands to produce more of the hormones adrenaline and cortisol and release them into the bloodstream. These hormones speed up heart rate, breathing rate, blood pressure, and metabolism. Blood vessels open wider to let more blood flow to large muscle groups, putting our muscles on alert. Pupils dilate to improve vision. The liver releases some of its stored glucose to increase the body's energy. And sweat is produced to cool the body. All of these physical changes prepare a person to react quickly and effectively to handle the pressure of the moment. This natural reaction is known as the stress response. Working properly, the body's stress response enhances a person's ability to perform well under pressure. But the stress response can also cause problems when it overreacts or fails to turn off and reset itself properly.

Strategies that restore immune tolerance are attracting growing interest in their potential to advance the treatment of type 1 diabetes beyond lifelong insulin therapy. Dan Jones investigates. In June, Bayhill Therapeutics and Genentech announced a collaboration to develop BHT-3021, a novel therapy to treat juvenile or type 1 diabetes (T1D). This autoimmune disease affects approximately 3 million people in the United States and there are 30,000 newly diagnosed cases a year — a figure that is rising by ~4% annually. “The numbers are increasing in epidemic proportions,” says Richard Insel, Executive Vice President of Research at the Juvenile Diabetes Research Foundation (JDRF), based in New York, which funded part of the early development of BHT-3021, a DNA plasmid that encodes proinsulin. Current treatment of T1D is centred upon the lifelong administration of the hormone insulin (by injection or pump), which patients are unable to produce in sufficient quantities because their immune system has destroyed the insulin-producing β-cells of their pancreas. Normally, insulin regulates blood sugar levels and metabolism, and lack of insulin leads to various complications over time, including eye disease and blindness, cardiovascular disease, and disorders of the kidneys and nervous system. Although insulin is life-saving, it is not a cure and cannot prevent the long-term complications. “We need to address the underlying immune response that caused the disease in the first place, and to restore β-cell function, to create a cure,” says Insel. However, “There is no approved way of stopping the autoimmune attack against the pancreatic β-cell in T1D thus far,” says Lawrence Steinman, Chair of the Interdepartmental Program in Immunology at the Beckman Center for Molecular Medicine, Stanford University, and co-founder of Bayhill. To address this issue, companies such as Bayhill have been developing therapies that use the immune system’s own mechanisms to ‘tolerize’ the autoreactive immune cells (Box 1). “Tolerogenic drugs re-establish tolerance to autoantigens by suppressing or inactivating pathogenic autoreactive T cells,” says Andrew Glasebrook, Senior Research Fellow at Lilly. “In a sense, these drugs re-establish the equilibrium between autoreactive T cells and T regulatory cells that has become disrupted.” For example, it is thought that BHT-3021 works in the following way: the proinsulin that is encoded by the DNA plasmid is processed by antigen-presenting cells of the immune system. They present the proinsulin to effector T cells as an antigen without the co-stimulatory signal that would ordinarily activate the effector cells to respond to the antigen. Instead, the lack of the co-stimulatory signal causes the effector T cells to become inactive against insulin, so that they no longer attack the insulinproducing pancreatic β-cells. Data on 50 patients with T1D who were treated with BHT-3021 in a Phase I–II trial show that it stabilizes C-peptide levels (a by-product of processing proinsulin to insulin) and decreases levels of glycosylated haemoglobin, which reflect blood sugar concentration over time (measurement of which is the main accepted surrogate end point of clinical trials for antidiabetic therapies). “We’ve shown we’re able to impact two very important parameters of the disease through this tolerogenic approach,” says Steinman. The new licensing deal between Bayhill and Genentech — in which Bayhill will receive an upfront payment of US$25 million and possible milestone payments of up to $325 million — will produce more detailed insights into the tolerogenic benefits of BHT-3021. “As we continue this trial and launch the more advanced trials, we’ll have a much better idea of the longevity of the effect and its potency,” says Steinman. BHT-3021 is not the only therapy in the T1D drug pipeline that aims to promote immune tolerance. The monoclonal antibodies (mAbs) otelixizumab (Tolerx/GlaxoSmithKline) and teplizumab (MacroGenics/Lilly), which target CD3 (a protein complex expressed by all T cells), are being evaluated in Phase III trials in T1D — and, as for BHT-3021, both received some early development funding from JDRF. “A tremendous amount of preclinical research evaluating anti-CD3 mAbs in the NOD [non-obese diabetic] mouse — a model of type 1 diabetes — demonstrates disease remission with a single course of treatment,” says Glasebrook. Human trials with CD3-specific mAbs are also showing promise, according to Doug Ringler, Chief Executive Officer of Tolerx, based in Cambridge, Massachusetts, which is the company co-developing otelixizumab with GlaxoSmithKline. “We’ve dosed well over 100 subjects to identify the most efficacious and safe dosing regimen to be used in the Phase III trial, which is ongoing,” says Ringler. Treatment causes a dramatic increase in the number and function of T regulatory cells, which control the effector arm of the immune system and suppress autoimmune attack on pancreatic β-cells, says Ringler, adding that these suppressive effects persist long after dosing has stopped and the drug has been fully metabolized. If the treatment is, as Ringler thinks, durable — lasting years, perhaps — it could dramatically reduce the severity of the disease, decrease reliance on administered insulin, improve metabolic control and preserve endogenous insulin production from any residual β-cells that are saved from destruction. “We think that ultimately this will result in a decrease in the incidence of vascular and other complications later in life,” says Ringler. However, there are some concerns about mAbs that target CD3. “They are highly specific, but they may hit an underlying molecule that has a broad effect,” says Steinman. CD3 is expressed by all T cells, he says, and so targeting this molecule can compromise the normal function of the immune system. “A published clinical trial [N. Engl. J. Med. 352, 2598–2608; 2005] of otelixizumab has shown re-activation of Epstein–Barr virus in recipients,” notes Steinman. Ringler acknowledges that immune-modulatory mAbs can cause serious side effects — as shown for the approved CD3-specific mAb muromonab (Orthoclone OKT3; Ortho Biotech) when used in transplantation therapy, and for the integrin-specific mAb natalizumab (Tysabri; Biogen Idec/Elan) when used to treat multiple sclerosis and Crohn’s disease. Nevertheless, Ringler says such safety concerns can be addressed through tailoring the dosage: “We can dose the antibody without any appreciable side effects, Epstein-Barr virus activation or cytokine release, and still be effective.” Although these Phase II dosing data have not yet been made public, they will be presented at the 20th World Diabetes Congress in Vienna, Austria, in October, says Ringler. Bo x 1 | Immune tolerance and its therapeutic potential Normally, during development, processes of central tolerance (which take place in the thymus) eliminate T cells that have high affinity for antigens that are created within the body (known as self antigens or autoantigens) so that the immune system recognizes only foreign antigens — typically indicating viral, bacterial or parasitic infections. Some potentially autoreactive T cells escape this central control mechanism. These T cells are kept in check by the process of peripheral tolerance, in which the cells are exposed to tissue antigens under non-inflammatory conditions, resulting in a tolerant state, or are actively suppressed by T regulatory cells. However, immune tolerance can go awry, particularly in the presence of stimuli that provide ‘danger’ signals, such as infection and tissue damage, leading to the activation of destructive autoreactive effector T cells. In the case of type 1 diabetes, this leads to the destruction of insulin-producing pancreatic β-cells. Therapies currently in development that aim to promote immune tolerance in T1D can be divided into two broad categories. The first exposes potentially autoreactive T cells to specific tissue autoantigens to which they should not respond (for example, with BHT-3021 the proinsulin is presented to the T cells) in the absence of a co-stimulatory signal. The second uses monoclonal antibodies (mAbs) that target CD3 (for example, otelixizumab and teplizumab), a protein complex expressed by all T cells that associates with the T cell receptor (TCR). It is postulated that when CD3-specific mAbs bind to CD3, in the short term, the antigen-specific pathogenic T cells undergo apoptosis or become anergic. In the long term, the numbers of adaptive regulatory T cells increase after treatment, which can control the pathogenic effector T cells (Nature Rev. Immunol. 7, 622–632; 2007). Some experts think that both CD3-specific mAbs and drugs such as BHT-3021 could have a role in the clinic, and combination therapies might prove particularly beneficial for patients who are newly diagnosed with T1D and still have some residual β-cell function (20–30% of normal). In these patients, shutting down the autoimmune response would preserve the remaining endogenous insulin production, which would ameliorate later complications of T1D. Insel suggests a possible scenario in which CD3-specfic mAbs are used when T1D is diagnosed to rapidly knock down the aberrant immune response that is destroying the β-cells, which could then be followed by BHT-3021 to achieve long-term immune tolerance. These immune-modulating approaches could, in turn, be combined with ways of restoring β-cell function, says Paul Burn, Broin Chair of The Sanford Project, a recently launched philanthropic initiative at Sanford Health and the Sanford School of Medicine, The University of South Dakota, that is focused on T1D. Glucagon-like peptide 1 (GLP1) and gastrin both stimulate β-cell proliferation, and a GLP1 analogue, exenatide (Byetta; Amylin/ Lilly), has already been approved for use in late-onset or type 2 diabetes. “Our translational R&D efforts include the design and execution of clinical trials that evaluate the combination of antigen-specific, tolerogenic approaches with GLP1- and gastrin-based regenerative approaches,” says Burn. Such an approach could boost endogenous insulin production in those with some residual β-cell function, and restore insulin production in those who have lost most of their β-cells (who would still require suppression of their autoimmune response to prevent destruction of regenerated β-cells). “Combining these approaches is the key to effectively treating both newly diagnosed and established T1D patients,” says Burn. “This is the future of T1D therapies and the first step on the path towards a cure.”
