Please fill out the form below as completely and accurately as possible. It takes about 15 minutes to complete. Submit the form at least two business days before admission to the hospital.
When you arrive at Sanford Health, we will ask you to verify your registration information. We'll make a copy of your insurance card, and you'll need to sign authorization forms.
