Health survey aims to identify community needs
Nobles County along with Sanford Worthington Medical Center have formed a Community Health Collaborative to conduct a community health needs assessment survey.
The survey aims to identify unmet health needs in the community, with results used to develop plans for addressing gaps in service.
“We are committed to the wellbeing of our community and ensuring we provide services to meet the needs of residents,” said Gwen Post, director of nursing and clinical services at Sanford Worthington.
Comprised of general health and wellness questions for residents in Nobles county, the survey is a voluntary way to address concerns in health care. All answers are confidential and no identifying information will be attained.
