Zip code: Enter the five digit zip code to search for Providers by a specific zip code. Providers with an address with this zip code will be displayed in the results. You can also search within a certain mile radius of this zip code if you enter the zip code along with a specified mileage in the "Show Providers Within" field.

City: Select a city from the drop down menu to search for Providers that are listed at a particular city. You can also search within a certain mile radius of this city if you select the city along with a specified mileage in the "Show Providers Within" field.

Show Provider Within - miles of your location: You must enter criteria in either fields labeled "Zip Code" or "City" to use this function. This field allows you to search within a specified range of a zip code or city.

Physicians: To search for all physicians within a specified location, click on the box next to "Physicians". If you would like to narrow your search, the following search options are available to you.

Physician Specialty: If you are searching for practitioners with a specific specialty, you can scroll through the list and click on the specialty you are looking for. You can select up to six different specialties to include in your results. If you want all specialties, do not click on any specific specialty and your results will pull all practitioner specialties.

Physician Last Name: If you know the exact spelling of the practitioner's last name you are searching for, you can enter it in this field and the database will search for practitioners with the specified last name only.

Gender: You have the option to narrow your search options by male, female, or both. Selecting male or female as part of your search will pull only those practitioners with that specific gender. By not selecting male or female specifically, your search results will pull all practitioners.

Hospital Affiliation: If you want to search for practitioners that have admitting and attending privileges at a Sanford Health Plan participating facility, enter the full or partial name you are looking for.

Medical Group Affiliation: To search for practitioners associated with specific medical group affiliation, enter the name in this field.

Accepting New Patients: If you would like to search for providers based on whether they are currently accepting new patients, you would select "Yes" from the drop down menu. By selecting "No", the results will pull practitioners who are practicing, but not currently accepting new patients. If nothing is selected here, all practitioners will be pulled whether they are accepting new patients or not accepting new patients.

Spoken Languages: All languages spoken by practitioners within the Sanford Health Plan network will be listed in the drop down menu. Select a specific language to locate practitioners that speak one of these languages.

Hospitals: To search for all Hospitals within a specified location, click on the box next to "Hospitals". If you would like to narrow your search, the following search options are available to you.

Hospital Name: To locate a specific hospital by name, enter all or a portion of the hospital name in this field. Leaving this field blank will display all hospitals within the specified range.

Ancillary Services (ex. skilled nursing facility, substance abuse/mental health facility, durable medical equipment, ambulatory surgery center etc.): To search for all Ancillary Services within a specified location, click on the box next to "Ancillary Services". If you would like to narrow your search, the following options are available to you.

Facility Type: If you are searching for providers with a specific service, you can scroll through the list and click on the type you are looking for. You can select up to six different services to include in your results. If you want all services, do not click on any specific types and your results will pull all ancillary services.

Facility Name: To locate a specified ancillary facility within a specified range, enter in the full or partial name.
