Improving the Care of Diabetic Patients with Hypertension
Improving the Care of the Diabetes Patients with Hypertension Activity is a structured process by which family medicine primary care physicians can review recommended practices for the assessment, documentation and treatment of hypertension in their patients with diabetes, retrospectively assess their management of hypertension in patients with diabetes and choose an improvement activity (Worksheet 1), apply the activity prospectively over a specified interval, and re-evaluate their performance (Worksheet 2).
- This activity has been approved by the American Board of Family Medicine as an external provider of Part IV Maintenance of Certification for Family Physicians credit.
- Term of approval is for two years from July 1, 2010, with the option for yearly renewal.
