Van Demark Symposium: Focus on MS & Parkinson's

Overall Purpose/Objective: To provide current information regarding the assessment, diagnoses, treatment, and management for MS and Parkinson’s patients. 

Syllabus: Syllabus will not be available at the symposium. The presentations will be emailed to you. Please print them and bring them with you.

Registration: Preregistration is required. Early Bird registration deadline is October 16, 2009. In the event that you are unable to attend the symposium after registering, please call (605) 328-6353 to cancel your registration.

Registration Fee: before 10/16/09 after 10/16/09
Physicians: $225     $275
Sanford Staff: $50     $50
Non-Sanford Staff: $175     $200
No charge for all healthcare students and residents.

Cancellation Policy: A full refund will be given for complete registration cancellations made before October 24, 2009. After October 24, 2009, a $50 cancellation fee will be assessed. A written letter requesting cancellation must be faxed to (605) 328-6301. No refunds will be issued for requests made after November 3, 2009.

Parking: This course is located at the Sanford USD Medical Center, 1305 West 18th Street, Schroeder Auditorium, Sioux Falls, SD 57117- 5039. The map outlines public parking on the Sanford campus. Valet parking is available on Friday.
