Congestive Heart Failure Disease Management

What is it?
The Congestive Heart Failure (CHF) Disease Management Program is designed for Sanford Health Plan members who have been diagnosed with CHF, and provides them with information about CHF and their lifestyle. The information is sent every other month and includes recipes, Little Books of Heart Wisdom, heart healthy education and references to additional information on CHF. It does not replace the direction and guidance of your doctor; however it compliments and reinforces the information received in the doctor’s office. The goal of the program is to provide tools to educate our members on promoting improved health through better prevention, detection, treatment and education.

Who is eligible?

All Sanford Health Plan, Sanford Health Plan of Minnesota members who have a diagnosis of CHF.

How do I join?

To participate in the program, print and complete the CHF Program Enrollment Form, and mail it to:
