Asthma Disease Management

What is it?
The Asthma Disease Management Program is a program designed for asthmatic members to provide them with information about asthma and their lifestyle. The information includes topics such as asthma triggers, asthma education, drug compliance, action plans, smoking cessation, an offer for a peak flow meter and spacer, and more. These mailings occur on a quarterly basis. It does not replace the direction and guidance of your doctor; however it compliments and reinforces the information received in the doctor’s office. The goal of the program is to provide tools to educate our members on promoting improved health through better prevention, detection, treatment and education.

Who is eligible?

All Sanford Health Plan, Sanford Health Plan of Minnesota members who have a diagnosis of asthma.

How do I join?

To participate in the program, print and complete the Asthma Program Enrollment Form, and mail it to:
