Breast Reconstruction

Under the Women's Health and Cancer Rights Act of 1998, health plans are required to cover breast reconstruction following a mastectomy. 

Sanford Health Plan of Minnesota covers reconstructive breast surgery and implants for those Members who had a mastectomy covered by this Plan, resultant from a disease, illness, or injury. SD and IA Members: For single mastectomy: coverage extends to the non-affected side to make it similar in size to the affected breast post-surgical reconstruction.

Includes two (2) external prosthesis per Calendar Year and two (2) bras per Calendar Year. For double mastectomy: coverage extends to two (2) external prosthesis per Calendar Year and two (2) bras per Calendar Year. MN Members: reconstructive breast surgery and implants are covered for those who had a mastectomy covered by this Plan resultant from a disease, illness or injury. This coverage extends to the non-affected side to make it similar in size to the affected breast post-surgical reconstruction. Includes two (2) bras per calendar year and replacements for external breast prosthesis or forms are limited to two (2) per affected side per calendar year.
