 What is it?
The Hypertension Disease Management Program is designed for members who have hypertension and provides them with information about hypertension and lifestyle. The information includes topics such as: causes, treatment, nutrition, exercise, medications and more. This program does not replace the direction and guidance of our healthcare practitioner, however it compliments and reinforces the information received in the office. The goal of the program is to provide tools to educate our members on promoting improved health through prevention, detection, treatment and education.
