Healthy Pregnancy Program

What is it?
The Healthy Pregnancy Program focuses on education and awareness. To participate, members must submit a health risk assessment in order for Sanford Health Plan to identify potential risks. Participants are then sent information based upon their answers. Members may also receive phone calls from a Health Plan nurse. The information provided through the program does not replace the care and guidance of your doctor; the goal of the program is to improve pregnancy outcomes through education and reinforce the care and education given at the doctor’s office.

Who is eligible?

Sanford Health Plan, Sanford Health Plan of Minnesota female members of all ages who are pregnant.

How do I join?

To participate in the program, print and complete the Healthy Pregnancy Enrollment Form, and mail it to:
