Community Care Program

Paying for healthcare can sometimes be a burden that becomes unmanageable. At times, it can even cause people to avoid seeking the care they need. We know that patients sometimes face life circumstances that makes paying for healthcare a challenge; we never want that to prevent them from obtaining healthcare services.

Our Community Care Program is here to ensure that all people receive the care they need, regardless of their financial situation, by providing discounted or free care to patients who meet the eligibility requirements.

What is Covered?

All Sanford services are eligible for the Community Care Program except for point-of-service co-pays, cosmetic procedures, non-covered services, non-covered accounts, eyewear, hearing devises (hearing aids) and other retail items.

Am I Eligible?

To qualify for the program, we will consider the following information:

  • You must reside in Sanford Health's service area in California, Iowa, Minnesota, Nebraska, North Dakota, Oklahoma or South Dakota.
  • Your ability to pay for required services.
  • Whether or not all other available payment resources, including medical assistance, have been exhausted.
  • Your financial information, showing your income, assets, expenses and supporting documentation.

How Can I Apply?

You may submit an application anytime before, during or after your healthcare services or hospitalization.

Applications are available by contacting a hospital financial counselor or any Sanford Patient Financial Services locations. Our financial counselors will be happy to assist you and are here to answer any questions you may have.

When Will I Know if I Qualify?

After you return your completed application to Sanford Patient Financial Services, we will review it and notify you of the decision by mail within 30 days.

What if I Do Not Qualify?

If you do not qualify, extended payment plans may be available. Please talk to a Financial Counselor to learn more.

Where Do I Submit My Application?
